What You Need To Do
You need to do 4 things:

1.  Pass a physical examPurple Hat will send you the necessary paperwork for your doctor to fill out.

2.  Have medical insurance.

3.  Sign a release of liability.  The wilderness can be a very dangerous place, what with lightening, snow storms, bears and other wild animals, exposed roots, and sand or rocks or pieces of wood on the ground that can cause you to trip or slip and fall, and lakes, just to name a few things.  You will be relieving Purple Hat and High Sierra Wilderness Fitness of any liability should you get injured or killed on the trip.  Purple Hat will send you the necessary paperwork for you to fill out.

4.  Get in shape to the extent that you can walk 5 miles at one time at your home.  While you probably live at a much lower elevation than where you will be backpacking, and that area will probably be flat, and you won't be carrying a backpack, Purple Hat has found that all he needs to do in terms of getting in shape before he leaves is be able to go out and walk 5 miles at one time.
